Memory Screen

Free and Anonymous Memory Screen
The Memory Screen takes 10-15 minutes to complete and is 94% sensitive in detecting memory impairment. At the completion of the screen a detailed report is generated suitable for sharing with your physician. You may use this screen to assess yourself or a loved one whom you know well.

20 Minute Time Out Policy
While completing the Memory Screen, if there is no activity for 20 minutes or longer (i.e. you take a break that is 20 minutes or longer, you take 20 minutes or longer to read and answer each question page, and so on), the system will time you out. Because this is an anonymous test and your responses are not saved, you will need to begin again.

Print or Save Your Report
Due to the anonymous nature of the screen, your results will not be saved. If you plan to review the results of the screen at a later date, please be prepared to print them out or save them. If you don’t have access to a printer, we encourage you to complete the screen regardless. You may return at a later date and complete a subsequent Memory Screen when you have access to a printer.
